Little League School Enrollment Form
 6/21/2017 This form can be used to verify league boundary eligibility in place of utility bills, etc. If used, this needs to be filled out and uploaded along with your child's registration.
SCLL Age Waiver Form
 1/3/2023 This form can be used to request a waiver for your child to play in a division higher than their league age. Your child will need to pass an age waiver assessment to ensure your child can safely participate in the higher division.
Medical Release Form   This form is completed by the player's parent or guardian and must be kept with the manager's safety binder throughout the season.
SCLL Spectator Code of Conduct
 3/26/2017 This is the Code of Conduct that ALL parents agree to when registering their child to play in our league. This code of conduct applies to league parents and their visitors.
Assessment Absence Request 2/6/2023 In compliance with Little League Regulation IV.F, any candidate failing to attend spring tryout sessions (assessment), shall forfeit league eligibility unless an excuse is presented which is accepted by a majority of the Board of Directors. If you are not able to attend assessments, you may fill out "Assessment Absence Request Form"

Baseball Pitch Count Log   This form is used to track individual pitches thrown by all pitchers during a specific game.
Baseball Pitcher Eligibility Log 9/7/2016 This form should be filled out following each game and kept with the safety plan.  It documents pitcher eligibility for baseball.
Softball Pitcher Eligibility Log 9/7/2016 This form should be filled out following each game and kept with the safety plan.  It documents pitcher eligibility for softball.

Injury Tracking Form
This form should be filled out any time there is an injury during an SCLL game or practice.  This information stays with SCLL and is tracked in accordance with our safety plan.
Accident Claim Form
(Instructions)		   This form is used for submitting an insurance claim to Little League International for expenses resulting from an injury during a game or practice.  It is secondary insurance meaning it will cover what your insurance provider doesn't such as deductibles, etc.  It will also cover you if you do not have insurance.

SCLL Draft Preference List
 2/1/2024 This form is used by managers who cannot attend the draft either in person or virtually. The manager will complete this sheet based on what they observed at the assessments and hand the sheet off to an approved proxy. The proxy will draft the team using the rankings on this sheet as a guide.
